Claim documents that need to be furnished include:

  • Photocopy of Discharge certificate/ card containing all relevant details from hospital
  • Photo ID proof and Address proof of the claimant
  • Cancelled cheque or Bank passbook copy of the claimant in order to get the following details.
    • Client Name
    • Bank Name
    • Bank branch name
    • Account number
    • MICR code
    • IFSC code
    • Account type
    • Bank phone number

Borrowers are required to make a written complaint and submit the same at the Branch or to the ADM of the Branch.
